(Code 38) Windows error when attach a pendrive or USB external hard drive

Symptom:
When you try to attach a pendrive or USB external hard drive, you get an error message: "Windows cannot load the device driver for this hardware because a previous instance of the device driver is still in memory. (Code 38)"
Windows doesn't recognize the pendrive or external HDD.

Solution:

Uninstall all USB devices from Device Manager, then reboot Windows and reinstall all USB device drivers (download the chipset driver from manufacturer support page and install it).
